Client Medication Profile
Hemophilia of Georgia pharmacy clients are asked to complete the Client Medication Profile form to help the pharmacy staff keep up with all of our clients' medications and allergies.
Remember to let the pharmacy know when your current medications or allergies change. Please call your HoG outreach nurse or the pharmacist or download the Client Medication Profile form and fax it back to the pharmacy at (770) 552-2810.
Remember to always tell each of your doctors about all the medications you take and about any changes in your medications prescribed by other physicians. Please feel free to make copies of the Client Medication Profile to give to your physicians and other pharmacies.
